Goal

Domus Medica's Health Guide is a questionnaire approved for population screening. The Health Guide asks citizens a number of standard questions. Based on the answers, health-promoting advice is given to the citizen. Moreover, the Health Guide is linked to a local offer that enables the citizen to immediately start implementing the advice in his/her region. 

Citizens can also voluntarily share the results of the Health Guide with their GP via the Electronic Medical Record. The GP can thus provide citizens with further advice to improve their lifestyle and health. 

This is a citizen-managed data platform. This means that every citizen has a personal digital safe deposit box on this platform and is the owner of his/her data. The use of the Health Guide over a longer period of time provides information about the change in health of the participants in order to improve their lifestyle. 

An advisory group provides feedback and discusses future visions on prevention from different perspectives: policy, care, companies and stakeholders.
